Â鶹ÊÓƵ’s patient portal is at . Your username is your email address. If you ever forget your password, you will need our practice ID to reset it: SZPHKT. Once you are logged on, you can securely:
– see all available appointment slots and book an appointment (Appointments)
– fill out your patient registration form before your first visit (REQUIRED: Chart > Forms)
– update new insurance information (REQUIRED: Chart > Patient Information > Additional Info)
– update your credit card on file (Billing > Manage Cards, deposit REQUIRED for some types of appointments)
– message the office securely with any follow up questions (Chart > Messages)
– add your children’s charts to your portal (Settings > Manage my patients)
FOR PRESCRIPTION REFILLS: Do not use the portal. It is easier and faster to use the Prescription Refill Form.
